Photograph of first attempt to fly the manned Langley Aerodrome, October 7, 1903. The experiment failed, and the machine and its pilot, Charles Manly, fell into the Potomac River. Manly was unhurt.

Samuel Pierpont Langley - Potomac experiment 1903. I do not know the origin of this photograph, nor whether it captures his first flight (Oct 1903) or second flight (Dec 1903). Nonetheless it clearly shows his launchpad aerodrome on the Potomac River. This image is in the public domain because first published before 1923.
